A text field that indicates the code that identifies the lab.

A check box that, when selected, indicates that this lab is located within your institution.

Note: If the lab is internal, users can track materials dispensed for the lab order. If the lab is external, users can track the invoice cost only.

A check box that, when selected, indicates the field is inactive and not in use.

A text field that describes the lab.

Text fields that indicate the address or location information for the lab.

The ellipses are used to display the City List window and State Codes window and select a city and state.

A text field that indicates the phone number of the lab.

A text field that indicates any notes or additional information about the lab.

1. Click the Clear Data () icon.
2. Complete the fields as necessary and click the Add a new Record () icon.

1. Select a listed entry.
2. Make any necessary changes and click the Modify Record () icon.

Note: Once a lab is created, the Code field cannot be modified.

1. Select a listed entry and click the Delete Record () icon.
2. When prompted, click Yes to confirm the deletion.

Note: You cannot delete a set code that contains stages. You must delete the stages first.
